About JourneyCare

JourneyCare is the largest nonprofit provider of hospice and palliative care in Illinois, serving nearly 3,000 adult and pediatric patients and their families each day, helping them to live fully, with dignity and without pain, on their own terms.

The agency was formed in 2015 as a result of the merger of three highly respected nonprofit community-based healthcare providers, Horizon Hospice & Palliative Care, JourneyCare, and Midwest Palliative & Hospice CareCenter, to offer more access to palliative, supportive and end-of-life care to patients and families across 10 counties in the Chicago area and northern Illinois.

Allstate Kid of the Week: Jan Gierlach
JourneyCare nominated Jan for this honor to highlight his upcoming Ride for 3 Reasons. On Feb. 7, 18-year-old Jan will embark on a 3,300 mile solo bike ride along the southern tier of the United States to raise funds and awareness for ALS and cancer research, along with hospice and advance care planning. JourneyCare is one of the local charities the Ride supports.
