How JourneyCare supports your team
- A designated interdisciplinary team of JourneyCare professionals – Registered Nurses, Certified Nursing Assistants, Social Workers, Spiritual Advisors and Integrative Therapists – works with your patient and their family, and with you and your staff.
- A dedicated JourneyCare Provider Relations Manager oversees and coordinates all aspects of the partnership.
- In-service training can be provided for your staff about topics related to hospice care and palliative medicine, pain management, communication, HIPAA and end-of-life issues.
- A physician’s profile is created that is accessible to your JourneyCare team members to ensure that your policies and communication needs are honored.
