What is JourneyCare Choices?
Through JourneyCare Choices, the Centers for Medicare and Medicaid Services (CMS) provides a new option for select Medicare beneficiaries to receive supportive care services typically provided by hospice while still receiving curative treatments from your primary care physician.
This new model is part of a nationwide initiative to deliver better care and put patients at the center of their care.
Is JourneyCare Choices Right for Me?
Those who are interested in receiving services under the JourneyCare Choices model must meet certain criteria. JourneyCare Choices is for someone with a diagnosis of advanced cancer, congestive heart failure (CHF) or chronic obstructive pulmonary disease. Even if you don’t meet the criteria for JourneyCare Choices, please ask about our other options to help you live your life on your own terms.
How Can JourneyCare Choices Help Me?
Under JourneyCare Choices, your primary care physician will continue to be in charge of your care. The JourneyCare Choices team will work together with your physician to provide supportive care services that can help you live as well as possible.
- Get the support you need, 24 hours a day, by managing and coordinating your care so you can live more comfortably at home without having to make additional trips to a doctor’s office;
- Maintain control of your health care by making sure you and your family have all the information you need to make informed decisions;
- Continue getting curative treatments while also receiving care focused on improving quality of life;
- Tolerate treatments for better outcomes.
What Services are Covered?
The supportive care and services provided through JourneyCare Choices are available by phone, 24 hours a day, seven days a week, anytime you need help. For those times when you need someone in person, we come to you.
Services provided by an interdisciplinary team of nurses, CNAs, social workers, chaplains and volunteers include:
- Care Coordination, Case Management and Shared Decision Making
- Pain and Symptom Management
- Social Work Services
- Spiritual care
- Volunteer Support
JourneyCare is one of only three hospices in the state of Illinois chosen to participate in Medicare Care Choices, a five-year clinical research trial allowing Medicare beneficiaries who meet eligibility criteria to simultaneously receive hospice care and curative treatment from their regular doctor and health care team.
