With 40 years of excellence and innovation, JourneyCare expands its community-based services for families living with serious illness.

Glenview. (June 3, 2019) JourneyCare, Illinois’ largest non-profit community-based hospice and palliative care provider, is pleased to announce it has purchased Northwestern Memorial HealthCare’s Home Health and Hospice programs.

This strategic opportunity expands JourneyCare’s ability to provide care across the continuum of serious illness and builds on its 40-years’ experience and expertise providing community-based advanced illness care. It also significantly grows JourneyCare’s service capacity in counties such as DeKalb, Kane, and DuPage and adds home health services to its family of services.

“Both organizations are mission-driven and committed to serve the community as non-profit providers,” said Sarah Boburka, JourneyCare’s President and Chief Executive Officer. “This is the right thing for patients and the community at large, giving more access and more specialized healthcare options to improve overall quality of life.” Due to an aging population and the rise of chronic illnesses, increasingly there is greater demand and consumer preference to remain independent at home. This is a tremendous opportunity for us to provide more services to support people living well in the community, added Boburka.

Through the purchase, JourneyCare will add home health care services to its expanding family of services which include: palliative care, hospice, care management, in-home primary care (House Calls), grief support and more.

JourneyCare is pleased to welcome the new team members.
