The Hope Foundation will award one (1) nursing scholarship. The student will receive a scholarship of up to $8000 for course and books.

Section A. Scholarship Requirements

  1. Must be a current full-time nursing student at Ambria College of Nursing.
  2. Must be graduating in May 2025 and receiving an associate’s degree in the Nursing Program.
  3. Must carry a minimum 3.0 GPA.
  4. Must have at least one full year of school remaining – Fall 2024/Spring 2025 Semester.
  5. Must be an IL Resident.

Section B. Applicants Must Provide the following information by 8/1/2024

  1. Completed Scholarship Application/Request fomr.
  2. Scanned copy of Official Transcript(s) from an accredited School of Nursing.
  3. Proof of Enrollment in Program and Cost.
  4. Written Essay – Minimum of one (1) page (max 3 pages) – Essay topics listed on Section C.
  5. Two (2) letters of professional recommendation attached to the application.

Section C. Scholarship Written Essay Instruction

  • The essay must be on any ONE of the below topics. Please note in your essay which topic you are addressing:
    • The effects of hospice care on the quality of life
    • Common misperceptions about hospice
    • The difference between palliative care and hospice
    • Explain the “goals of care” and why it is important for healthcare professionals to have this conversation with their patients with a serious illness
    • Explain the members of the hospice care team and their roles in providing end-of-life care
  • The essay must be no longer than 3 pages double-spaced, 12-point font.

Scholarship finalists will be required to complete an interview with a member of the Addus Scholarship Committee.

Section E. Terms and Conditions

The scholarship will be paid to the university or college in the name of the recipient for student-related expenses. The scholarship will be provided in two payments. The first payment will be issued to assist with the Fall Term of 2024. The second payment will be issued for the Spring Term of 2025 after we receive proof of a minimum 3.0 GPA, and proof of full-time enrollment in Spring 2025.

The Scholarship Recipient will participate in a 16-hour mentorship with members of the local Addus hospice team prior to nursing program graduation. This mentorship is expected to be completed by 3/31/2025. 

The Scholarship Recipient agrees to sign a Media Release to create and publish internal and external publications.
